About the Opportunity

We are seeking a Physical Therapist Assistant to join a supportive skilled nursing facility (SNF) rehabilitation team in Sacramento, CA. Working under the supervision of a licensed Physical Therapist, you will carry out individualized treatment plans and help patients progress toward meaningful functional goals.

Responsibilities

• Implement treatment plans established by the supervising Physical Therapist.
• Provide therapeutic exercise, gait, balance, mobility, and functional training.
• Monitor patient response and communicate progress to the Physical Therapist.
• Educate patients and caregivers within the established plan of care.
• Complete accurate and timely documentation.
• Collaborate with the rehabilitation and interdisciplinary care team.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active CA PTA license in good standing, or eligibility for licensures.
• Current BLS/CPR certification as required by the employer.
• Strong communication, documentation, teamwork, and patient-care skills.
• Experience in the applicable setting preferred.
