Job Title: Geriatric Wound Care Nurse Practitioner (AGNP/CNP) | Mobile Rounding Provider

Company: Twin Cities Physicians

Location: Greater Twin Cities Metro Area, MN

Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Schedule: Full-Time | Monday through Friday

You'll work directly inside assisted living communities, memory care settings, skilled nursing facilities, and long-term care environments throughout the Twin Cities Metro, bringing specialized care directly to patients where they live.

What You'll Be Doing

  • Provide advanced wound care assessments, treatment planning, and ongoing management.
  • Deliver on-site primary care and rounding services throughout senior living communities.
  • Manage pressure injuries, diabetic wounds, post-surgical wounds, and complex chronic conditions.
  • Coordinate care with physicians, nursing teams, therapists, caregivers, and facility leadership.
  • Support preventative care initiatives, chronic disease management, and continuity of care.
  • Help improve healing outcomes while reducing avoidable hospitalizations and complications.

Qualifications

  • Active Minnesota Certified Nurse Practitioner (CNP) license.
  • Experience in wound care, geriatrics, primary care, post-acute care, or related settings preferred.
  • Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
  • Strong clinical judgment and confidence managing medically complex patient populations.
