We are seeking a talented Community Case Monitor II to join our team of nearly 500 employees. CCSI is contracted by Onondaga County to provide program management services, including specialized staffing. The Community Case Monitor II will be located in Onondaga County and have a primary responsibility of monitoring all aspects of Enhanced Voluntary Agreements (EVA) as well as AOT Court Orders as necessary. The position will carry a caseload and provide support and oversight for high-needs individuals with serious mental illness who have a history of medication non-compliance and who live in Onondaga County. The Community Case Monitor II will coordinate and facilitate multi-disciplinary teams that include: the client, behavioral health treatment providers, case management, housing supports, and other community based organizations as outlined in the treatment plan. The Community Case Monitor II will support management to mentor Community Case Monitor I staff on community resources and relationships. Community Case Monitor II will assist in supporting the evaluation process for individuals residing in the community. This role will be responsible for tracking, data collection, tool creation and assigned parts of reporting process. Supervision will be provided by the Forensic Case Monitor Supervisor, in close collaboration with the AOT Coordinator and Director of Forensic and Outreach Services.

CCSI is partnering with Onondaga County  to hire a/an Community Case Monitor II. This position is employed by CCSI and working on behalf of Onondaga County.

In this role, you will

  • Mentor and support Community Case Monitor I informally and formally in addressing complex client needs and develop relationships with community providers.
  • Provide physical and technical assistance to the completion of evaluation process for individuals in the community.
  • Assist Forensic Case Monitor Supervisor in collecting, documenting, and analyzing monthly and quarterly data within the AOT program.
  • Initiate case reviews.
  • Support program level data development, tracking, and reporting.
  • Assess needs of referred and assigned individuals.
  • Coordinate the development of system-wide treatment and support plans to include treatment services, case management, housing, transitional support services and other identified supports.
  • Monitor client’s participation in and compliance with an established treatment plan.
  • Schedule, plan, and facilitate team meetings.
  • Communicate effectively with the client, providers, families, and criminal justice personnel to assist with developing and sustaining the plan.
  • Create a sustainable treatment team by assigning roles and tasks to the treatment team to prepare for the transition to a lower level of care. 
  • Monitor treatment implementation and coordinates action when treatment, service, and support plan needs adjustment.
  • Recommend adjustments to treatment plans, and advocate for system movement.
  • Track individuals across multiple systems of care over time to assure continuity of linkages and supports.
  • Work closely with treatment providers, ACT, FACT, case management, housing providers, parole, probation, Peer Bridger, and unique services and supports.
  • Culturally responsive to the population being served in the development, design, monitoring, evaluation, and/or implementation of programs and services.
  • Record progress notes in a comprehensive, timely manner.
  • Organize interviews and/or meetings with referred individuals and providers involved in the plan as needed.
  • Cultivate partnerships with community providers to best support clients.
  • Supports community education requests.
  • Request and review clinical records as needed. 
  • Initiate, assist in, and complete investigations for appropriateness of AOT Court Orders and EVAs.
  • Attend staff and other community meetings.
  • Maintain and organize electronic and paper records, update tracking sheets, and maintain files for the program, including archiving records.
  • Identify creative solutions to complex problems utilizing available community resources
  • Support clients in achieving their program goals.
  • Develop, track, and report program goals and objectives.
  • File maintenance for the program, including filing and archiving of records.
  • Travel as necessary to fulfill responsibilities.
  • Perform other duties as assigned.

What you bring

  • Bachelor’s degree in Human Services field plus five years experience in mental health required (see CCSI equivalency chart).
  • Applicant with prior experience or knowledge of AOT preferred.

  • A working knowledge of systems of care (i.e. OMH Housing, Chemical Dependency, Criminal Justice, Homeless, Health Care).
  • Ability to prepare effective written correspondence, records, and reports.
  • Good powers of observation, analysis, and negotiation.
  • Ability to work in a person-centered way with clients, families.
  • Ability to work collaboratively with individuals, families, law enforcement, the court system, state agencies, state forensic services, and local service providers.
  • Ability to facilitate multi-disciplinary team discussions or meetings.
  • Ability to work with minimum direction and supervision.
  • Ability to plan, schedule and facilitate complex cross systems meetings.

  • Respect the confidential nature of certain information you are exposed to in the course of your work performance.      Staff working directly with protected health information /electronically-stored protected health information, personally identifiable information, and other patient/consumer/customer data will abide by the specific procedures and policies outlined by CCSI’s Code of Conduct and CCSI’s Privacy and Security Policies, New York State regulations and Federal regulations.
  • Must have a valid NYS driver’s license and reliable transportation. 
  • Satisfactory completion of all required background screenings: criminal, and driving.
