Compensation: $60 - $70 per hour

About the Opportunity

We are seeking a Physical Therapist to join a collaborative skilled nursing facility (SNF) team in Los Altos, CA. This opportunity is ideal for a clinician who enjoys helping patients improve mobility, strength, balance, safety, function, and independence through individualized, evidence-based care.

Responsibilities

  • Evaluate patients and develop individualized plans of care based on functional goals.
  • Provide evidence-based therapeutic exercise, mobility, balance, gait, and functional training.
  • Monitor patient progress and modify treatment plans as clinically appropriate.
  • Educate patients, families, and caregivers on safety and home programs.
  • Complete accurate and timely clinical documentation.
  • Collaborate with physicians, nursing, rehabilitation professionals, and the interdisciplinary team.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Active CA Physical Therapist license in good standing, or eligibility for licensures.
  • Current BLS/CPR certification as required by the employer.
  • Strong clinical reasoning, communication, documentation, and teamwork skills.
  • Experience in the applicable setting preferred; qualified clinicians at varying experience levels are encouraged to apply.
