About Groundworks Behavioral Health
Greater Georgia Behavioral Health is launching Groundwork, a new Adolescent Afterschool Intensive Outpatient Program (IOP) based in Fayetteville, GA. The program will serve adolescents ages 12 to 17 through individual, group, and family therapy, drawing on evidence-based, trauma-informed approaches including DBT-A, CBT, trauma-focused work, and family systems.
The Opportunity
Groundworks Behavioral Health is seeking a part-time Case Manager to support adolescents receiving behavioral health services in Fayetteville, Georgia. This evening position focuses on building supportive relationships, coordinating care, and helping clients navigate services and resources that support their treatment goals. The position offers $20 - $24 per hour and an opportunity to contribute to a collaborative behavioral health environment.
What You'll Do

• Build positive, supportive relationships with adolescent clients while maintaining appropriate professional boundaries.
  
• Assist clients with identifying needs, accessing available resources, and coordinating supportive services.
  
• Work collaboratively with clinical and operational team members to support continuity of care.
  
• Help clients understand and follow through with their individualized treatment and service plans.
  
• Maintain accurate and timely case management documentation and communicate relevant updates to the appropriate team members.
  
• Support communication and coordination between clients, families, and treatment team members as appropriate.
  
• Provide consistent, compassionate case management support during scheduled evening hours.
  

Requirements

Required

• High school diploma or GED.
  
• Entry-level candidates are welcome; less than one year of relevant experience is acceptable.
  
• Strong interpersonal and communication skills, with the ability to establish professional, supportive relationships with adolescent clients.
  
• Ability to coordinate services, maintain organized records, and communicate effectively with a multidisciplinary team.
  
• Ability to maintain confidentiality, professional boundaries, and appropriate documentation standards in a behavioral health setting.
  
• Availability to work part-time evening shifts on-site in Fayetteville, Georgia.
