Director of Patient Care Services Opportunity:

• Full-Time, Permanent, Direct-Hire
  
• Schedule: Monday - Friday, 8 AM - 5 PM
  
• Setting: Home Health

Director of Patient Care Services Responsibilities:

• Maintain adherence to state regulations, federal laws, and Medicare Conditions of Participation.
  
• Direct patient care plans, admissions, re-certifications, and multi-disciplinary case conferences.
  
• Lead a team of clinical staff and administrative support personnel. Provide leadership, guidance, and mentorship to ensure high-quality patient outcomes and strong team collaboration.
  
• Oversee documentation accuracy, survey readiness, and interdisciplinary team communication.
  

Director of Patient Care Services Qualifications:

• Must have an active and unencumbered RN License
  
• Must have at least 4 years of recent home health experience
  
• Must have previous management-level experience in home health, ideally at the manager, supervisor, or director level.
