Job Title: Hospice RN Float | Admissions, Case Management & Field Visits

Company: Superior Hospice

Location: Greater Twin Cities Metro Area | 21-County Service Area

Compensation: $90,000-$130,000 annually, depending on experience

Benefits: Comprehensive Benefits Package

Schedule: Monday-Friday with On-Call and Weekend Rotation

What You'll Do

  • Complete hospice admissions, eligibility evaluations, informed consents, education, and initial care planning.
  • Provide case management support as needed, including symptom management, medication oversight, and ongoing care coordination.
  • Complete routine, PRN, urgent, and crisis field visits throughout the Twin Cities metro area.
  • Collaborate closely with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers to provide exceptional interdisciplinary care.
  • Educate patients and families on disease progression, comfort-focused care, and hospice services.
  • Maintain timely, accurate EMR documentation while ensuring compliance with Medicare Conditions of Participation and hospice regulations.
  • Adapt quickly to changing clinical priorities while delivering compassionate, patient-centered care.

What We're Looking For

  • Active Minnesota Registered Nurse (RN) license.
  • Hospice experience preferred, or strong clinical experience in ICU, oncology, palliative care, home health, emergency medicine, or other complex care settings.
  • Excellent assessment skills, critical thinking, and independent clinical judgment.
  • Comfortable managing changing priorities while maintaining exceptional patient care.
  • Outstanding communication and relationship-building skills with patients, families, and interdisciplinary teams.
  • Reliable transportation and willingness to travel throughout our service area.
  • A compassionate nurse who enjoys learning, problem-solving, and making a difference wherever needed.
