We are seeking a Physical Therapist to join a collaborative skilled nursing facility (SNF) team in Fresno, CA. This opportunity is ideal for a clinician who enjoys helping patients improve mobility, strength, balance, safety, function, and independence through individualized, evidence-based care.

Responsibilities

·        Evaluate patients and develop individualized plans of care based on functional goals.

·        Provide evidence-based therapeutic exercise, mobility, balance, gait, and functional training.

·        Monitor patient progress and modify treatment plans as clinically appropriate.

·        Educate patients, families, and caregivers on safety and home programs.

·        Complete accurate and timely clinical documentation.

·        Collaborate with physicians, nursing, rehabilitation professionals, and the interdisciplinary team.

Qualifications

·        Graduate of an accredited Physical Therapy program.

·        Active CA Physical Therapist license in good standing, or eligibility for licensures.

·        Current BLS/CPR certification as required by the employer.

·        Strong clinical reasoning, communication, documentation, and teamwork skills.

·        Experience in the applicable setting preferred; qualified clinicians at varying experience levels are encouraged to apply.
