We are seeking a hands-on, experienced EMR & CRM Migration Project Manager to lead a technology transformation engagement for our client. The client's service lines span private-pay personal care, skilled home health, and assisted-living placement, with limited exposure to public payors.

The client has recently selected a new EMR platform and needs an experienced provider to lead the full migration from its legacy system — including the transfer of all historical data, system reconfiguration, and redesign of supporting workflows — while coordinating the EMR with the company's CRM and data-warehouse environment so leads can be tracked across the entire care lifecycle. This is a hands-on, embedded engagement rather than a hands-off advisory role.

Candidates with WellSky-to-AxisCare migration or a WelcomeHome-to-AxisCare transitions are strongly preferred.

What You Will Do

• Migration Leadership: Own end-to-end migration from the client's legacy EMR to its newly selected platform, including the transfer of all historical data, system reconfiguration, and go-live readiness.
• Workflow & Playbook Redesign: Rebuild the EMR configuration and modernize the franchise playbook and supporting workflows, which no longer reflect the current complexity of the business.
• CRM & Data Warehouse Integration: Coordinate the EMR platform with the company's CRM and data-warehouse environment so leads and clients can be tracked seamlessly across the full care lifecycle.
• Change Management & Rollout: Design and lead change management for the revised operating model, delivering a thoughtful, phased rollout across the 115-location franchise network with dedicated transition support.
• Stakeholder Coordination: Serve as the hands-on point of contact between corporate leadership, franchise operators, and EMR/CRM vendor teams to keep data migration, configuration, and rollout on track.

Ideal Profile

• The Hands-On Implementer: Comfortable working deep in configuration, data migration, and workflow detail — not just directing from a distance — with a personal, boutique style suited to a PE-backed, fast-moving environment.
• The Home Care Technology Expert: Has led EMR and/or CRM transitions specifically within home care, personal care, or franchise healthcare settings, and understands the operational nuance across private-pay personal care, skilled home health, and assisted-living placement.
• The Change Leader: Skilled at translating a redesigned operating model into a rollout that a nationwide, generally tech-mature franchise network can adopt smoothly, with the right level of dedicated support.
• The PE-Savvy Advisor: Experienced supporting PE-backed businesses, comfortable with the pace, reporting rigor, and budget discipline that come with that environment.

Requirements

Background: Demonstrated experience leading EMR migrations — including full historical data conversion, system reconfiguration, and workflow redesign — within home care, personal care, or similar franchise-based healthcare services organizations.

• CRM & Data Integration: Experience coordinating EMR platforms with CRM and data-warehouse systems to support lead and client tracking across a full care lifecycle.
• Change Management: Proven track record designing and leading change management and phased rollout plans across multi-location or franchise networks.
• PE Environment Experience: Prior experience supporting private-equity-backed companies, with comfort operating under scope-driven, middle-market budget expectations.
• Availability: Able to begin promptly, with capacity to support an engagement anticipated to run approximately one year.
