Job Summary:

We are looking for an adaptable Post Acute Primary Care Physician (MD/DO) to oversee our post-acute clinical services across two key locations: Melbourne and Merritt Island, FL. This position focuses on delivering exceptional, hands-on care through daily rounds at our partner Skilled Nursing Facilities (SNFs). You will serve as the primary clinical leader on-site, managing higher-risk patients and supervising a dedicated team of Nurse Practitioners and Physician Assistants.

Schedule:

• Hours: Monday to Friday, 8:00 AM – 5:00 PM
  

Key Responsibilities:

• Conduct daily rounds splitting coverage between two facility locations in Melbourne and Merritt Island.
  

• Perform initial assessments and evaluations for all newly admitted patients.
  

• Manage acute updates and coordinate care plans for higher-risk, fragile patients.
  

• Monitor, supervise, and collaborate closely with regional Advanced Practice Providers (APPs).
  

Qualifications & Requirements:

• Board Certified (BC) or Board Eligible in Family Medicine or Internal Medicine (Board Certification is NOT a requirement).
  

• Active Florida Medical License.
  

• ACN (Area of Critical Need) license holders are accepted and welcome to apply.
  

• Ability to commute seamlessly between facilities in Melbourne and Merritt Island.
  

Compensation & Benefits:

• Base Salary: $225,000 – $245,000 per year
  

• Time Off: 2 weeks (14 days) of Paid Time Off (PTO)
