A fast-growing healthcare organization that expands access to care for older adults in underserved communities is looking to hire a Nurse Practitioner or Physician Assistant. Advanced Practice Providers lead its value-based model, which prioritizes quality, outcomes, and meaningful patient relationships over visit volume.

Through senior-focused clinics and partnerships with health plans and provider groups, the organization improves health outcomes, reduces avoidable healthcare costs, and creates a better patient experience.

Join a supportive clinical community backed by care coordinators, physician advisors, and other healthcare professionals. In this role, you will expand access to high-quality care and help older adults navigate complex health challenges with confidence.

What You’ll Do

As a Nurse Practitioner or Physician Assistant, you will care for medically complex older adults in outpatient clinics and their homes. You will work as an embedded member of an interdisciplinary care team to manage chronic conditions, support transitions from the hospital, address psychosocial needs, and prevent avoidable acute-care admissions.

You will conduct Medicare annual wellness visits, review medical histories and medications, perform screenings, develop care plans, and educate patients and families. You will also complete HCC coding and risk-adjustment documentation, coordinate follow-up care, and maintain timely, accurate medical records.

The role typically involves 10–12 clinic visits or 5–6 home visits per day. Home visits take place within approximately 30 miles of the Salem region. You must complete visit documentation within 72 hours.

Schedule Highlights

  • Full-time, Monday–Friday, 8:00 a.m.–5:00 p.m.

Requirements

  • Current Oregon Nurse Practitioner license and ANCC or AANP certification

  • Knowledge of chronic-care management, medication options, Medicare HCC coding, and managed care
