Duties & Responsibilities

• Provide comprehensive primary care to elderly patients in clinic and community settings.
  
• Perform home visits to assisted living, SNF, and personal residences.
  
• Collaborate with interdisciplinary team (11 members) to coordinate patient care.
  
• Manage patient care via Epic EMR.
  
• Participate in PACE program, focusing on holistic care for over-55 population.
