Job Title: Home Health Admissions Nurse (RN) | The Clinical Leader Every Patient Meets First

Compensation: $150–$200 per completed visit + Mileage Reimbursement

As an Admissions RN, you'll serve as the clinical leader who ensures every patient begins home health with confidence, clarity, and the right plan for success.

What You'll Do

  • Complete comprehensive Start of Care (SOC) admissions throughout the Twin Cities Metro.
  • Perform detailed nursing assessments and complete accurate OASIS documentation.
  • Develop individualized plans of care that set patients up for long-term success.
  • Educate patients and families on diagnoses, medications, services, and expectations.
  • Coordinate seamlessly with physicians, case managers, therapists, and the interdisciplinary care team.
  • Ensure smooth, safe transitions from hospitals, skilled nursing facilities, and clinics into home health.

What We're Looking For

  • Active Minnesota Registered Nurse (RN) license.
  • Strong clinical assessment and critical thinking skills.
  • Home health and OASIS experience strongly preferred.
  • Exceptional communication and patient education abilities.
  • Ability to work independently and manage a field-based schedule.
  • Reliable transportation.
