Job Purpose

To provide high-quality nursing care to the practice population, promoting health and

wellbeing through assessment, treatment, screening, health education, and chronic

disease management. The post holder will work collaboratively with the multidisciplinary

team to deliver safe, effective, patient-centred care and contribute to the achievement of

practice clinical and quality objectives, including Quality and Outcomes Framework

(QOF) targets.

The post holder will provide high-quality nursing care within both the practice and

sometimes in a community setting, including undertaking home visits for patients who

are unable to attend the surgery.

Key Responsibilities

Clinical Practice

The Practice Nurse will:

 Deliver evidence-based nursing care within their scope of practice.

 Undertake patient assessments, treatment, and evaluation of care.

 Maintain accurate and contemporaneous clinical records.

 Identify and manage patients at risk and escalate concerns appropriately.

 Adhere to practice policies, professional standards, and clinical guidelines.

 Promote equality, diversity, and patient-centred care throughout all aspects of

service delivery.

Home Visits

The Practice Nurse will:

 Undertake home visits for housebound patients and those unable to attend the

practice when clinically appropriate.

 Assess, plan and deliver nursing care within the patient's home environment.

 Monitor and manage long-term conditions during home visits where appropriate.

 Undertake clinical procedures including wound care, medication monitoring,

observations and other delegated nursing interventions within scope of practice.

 Identify and escalate concerns regarding a patient's physical health, mental

wellbeing, safeguarding needs or home circumstances.

 Maintain accurate and contemporaneous clinical records following each visit.

 Liaise effectively with GPs, community services, secondary care providers and

other members of the multidisciplinary team to ensure continuity of care.

Long-Term Condition Management

The Practice Nurse will:

 Undertake annual reviews and ongoing management of patients with long-term

conditions.

 Manage nurse-led clinics for:

• Diabetes
• Asthma
• COPD
• Hypertension
• Coronary heart disease
• Heart failure
• Stroke and TIA
• Atrial fibrillation

 Monitor clinical outcomes and support achievement of QOF indicators.

 Develop and review personalised care plans.

 Support medicines optimisation and adherence.

 Refer patients to other healthcare professionals and services where appropriate.

Respiratory Care

The Practice Nurse will:

 Conduct annual asthma and COPD reviews.

 Assess asthma control and exacerbation history.

 Assess and record inhaler technique.

 Provide self-management education and personalised action plans.

 Promote smoking cessation and healthy lifestyle changes.

Diabetes Management

The Practice Nurse will:

 Conduct diabetic annual reviews.

 Perform and record diabetic foot assessments.

 Monitor HbA1c, blood pressure and cardiovascular risk factors.

 Provide patient education to support self-management.

 Support patients with treatment plans and lifestyle modifications.

Health Promotion and Disease Prevention

The Practice Nurse will:

 Promote healthy lifestyles and preventative healthcare.

 Provide advice regarding:

• Smoking cessation
• Weight management
• Diet and nutrition.
• Physical activity
• Alcohol consumption

 Support national and local screening programmes.

 Encourage patient participation in preventative health initiatives.

Immunisations and Vaccinations

The Practice Nurse will:

 Administer childhood and adult immunisations in accordance with national

guidance.

 Deliver seasonal vaccination programmes.

 Maintain vaccine storage, cold chain management, and documentation

requirements.

 Ensure accurate clinical coding and recording of vaccinations.

Cervical Screening

The Practice Nurse will:

 Undertake cervical cytology screening in accordance with national standards and

training requirements.

 Support patient education and informed consent.

 Ensure results are appropriately managed and followed up.

Clinical Procedures

The Practice Nurse will be competent and appropriately trained to undertake:

 Venepuncture.

 Blood pressure monitoring.

 NHS Health Checks.

 Urinalysis.

 Height, weight, and BMI assessments.

 Wound care and dressings.

 Ear irrigation or microsuction (where trained).

 Other delegated clinical procedures appropriate to the role.

Quality and Outcomes Framework (QOF)

The Practice Nurse will:

 Support the achievement of QOF targets through effective patient management

and accurate clinical coding.

 Participate in recall and review systems for long-term conditions.

 Ensure clinical activity is coded accurately and in a timely manner.

 Identify patients requiring review and support completion of clinical indicators.

 Contribute to audits and quality improvement initiatives.

Clinical Administration

The Practice Nurse will:

 Maintain accurate patient records using the practice clinical system.

 Process and action clinical information in accordance with practice protocols.

 Contribute to effective communication within the multidisciplinary team.

 Participate in the development and review of practice protocols and procedures.

Safeguarding and Governance

The Practice Nurse will:

 Act in accordance with safeguarding policies and procedures.

 Report concerns relating to vulnerable adults and children.

 Always maintain patient confidentiality.

 Comply with Information Governance and Data Protection requirements.

 Participate in incident reporting, significant event reviews, and clinical

governance activities.

Professional Development

The Practice Nurse will:

 Maintain NMC registration and professional competence.

 Participate in annual appraisal and personal development planning.

 Undertake mandatory and role-specific training.

 Keep up to date with changes in clinical practice, national guidance, and primary

care developments.

Person Specification

Essential

 Registered Nurse with current NMC registration.

 Experience of working in a primary care or community healthcare setting.

 Knowledge of long-term condition management.

 Experience of delivering immunisations and vaccinations.

 Excellent communication and interpersonal skills.

 Ability to work independently and as part of a multidisciplinary team.

 Competent IT skills and experience of electronic clinical systems.

Desirable

 Qualification in cervical cytology.

 Chronic disease management qualifications.

 Respiratory disease management training.

 Diabetes management qualification.

 Independent prescribing qualification.

 Experience of EMIS.

 Experience of QOF management and clinical coding.
