Licensed Practical Nurse ( LPN)

Position Summary
The LPN owns the patient's first hands-on touch in the treatment visit at Valley Vital Care, from rooming and vitals to patient education and infusion suite readiness. This role carries the former Patient Educator duties, ensuring patients are prepared, informed, and moving through the suite efficiently — keeping the day on time without compromising care. education depth, and leadership.
Core Responsibilities
All LPN's are responsible for the following:

• Room patients, take vitals, and prepare them for treatment.
  
• Deliver patient education and onboarding support per protocol.
  
• Own infusion suite readiness: setup, turnover, and supplies.
  
• Own delegated, non-clinical support tasks for nursing within scope.
  
• Document accurately in CareTend and the EHR.
  
• Own follow-up and reminder outreach that keeps care on schedule.
  
• Ensure a clean, stocked, and safe patient environment.
  
• Escalate clinical concerns to nursing promptly.
  

Qualifications:

• High school diploma or equivalent
  
• LPN State Certification required
  
• BLS certification
  
• 0 to 1 year clinical support experience
