Nurse Practitioner

We are seeking a compassionate and motivated Nurse Practitioner to provide wound care services to patients in skilled nursing facilities and nursing homes across the Rome, Utica, Syracuse, and Watertown areas. This is an excellent opportunity for a provider who enjoys working independently, values flexibility, and wants to make a meaningful impact in long-term care settings.

We are open to full-time or part-time candidates. Part-time providers may cover a portion of the facilities based on availability and preference.

Responsibilities

• Provide wound care evaluations and follow-up visits in nursing home settings
  
• Assess, diagnose, and manage acute and chronic wounds
  
• Develop and implement individualized treatment plans
  
• Collaborate with nursing facility staff and interdisciplinary care teams
  
• Document patient assessments, progress notes, and treatment recommendations
  
• Educate staff on wound prevention, healing, and best practices
  
• Ensure compliance with applicable standards of care and documentation requirements
  

Qualifications

• Active Nurse Practitioner license in New York State
  
• Strong clinical assessment and patient care skills
  
• Wound care experience preferred, but not required
  
• Willingness to train in wound care for candidates with the right background and interest
  
• Excellent communication and time management skills
  
• Ability to travel between assigned facilities
  

Compensation & Benefits

• Annual salary range: $140,000–$170,000, based on experience and schedule
